18F-Fluorodeoxy-Glucose Positron Emission Tomography Pattern and Prognostic Predictors in Patients with Anti-GABAB Receptor Encephalitis

Rationale: To identify the metabolic pattern and prognostic predictors in anti-gamma-aminobutyric-acid B (GABAB) receptor encephalitis using 18F-fluorodeoxy-glucose positron emission tomography (18F-FDG-PET).

Methods: A total of 16 patients with definite anti-GABAB receptor encephalitis who underwent 18F-FDG-PET were retrospectively identified. 18F-FDG-PET images were analyzed with comparison to controls using a semi-quantitative statistical parametric mapping method. Clinical outcomes were assessed by the modified Rankin Scale. Group comparisons based on 18F-FDG-PET data were made between patients with good outcomes and poor outcomes.

Results: Initial 18F-FDG-PET was abnormal in 13 of 16 patients (81.3%), with 11 patients (68.8%) demonstrating hypermetabolism in the medial temporal lobe (MTL). Furthermore, 18F-FDG-PET was more sensitive than MRI in the diagnosis of anti-GABAB receptor encephalitis (81.3% vs 37.5%, P < 0.05). After a median follow up of 24.5 months (IQR: 26.8 months, range: 6-63 months) since symptom onset, 10 of 16 patients (62.5%) showed good outcomes, whereas poor outcomes were observed in 6 patients (37.5%). The group comparisons based on voxel-based analysis showed that patients with poor outcome demonstrated increased metabolism in the MTL compared to controls group (FWE corrected, P < 0.05). In addition, patients with poor outcome also exhibited higher metabolic cluster in the MTL than those patients with good outcome (uncorrected, P < 0.001).

Conclusions: 18F-FDG-PET was more sensitive than MRI in the diagnosis of anti-GABAB receptor encephalitis. MTL hypermetabolism detected by brain 18F-FDG-PET may serve as a precise biomarker for evaluating the long-term outcome in patients with anti-GABAB receptor encephalitis.
