Diastat Gel for Acute Seizures: A Parental Survey

RATIONALE: Optimal management for acute seizure control requires a safe, effective and readily available treatment modality which can be easily administered by medical, paramedical and non-medical individuals. While intravenous benzodiazepines are effective and readily available, their administration requires medical or paramedical personnel. The use of rectal diazepam has allowed for safer administration by a broader range of people including family members. METHODS: We have conducted a retrospective phone survey of people living outside city limits to assess parental comfort with the home administration of Diastat gel(a prefilled unit-dose rectal delivery system of diazepam)to children prone to episodes of acute prolonged seizures or seizure clusters. Sixteen families who have used Diastat gel were surveyed with regards to their comfort administering the medication and perceived efficacy of the treatment. RESULTS: Fourteen of the 16 surveyed were completely comfortable administering Diastat gel. In eight (50%) there was complete cessation of seizure activity after one dose while the remainder required a second dose. Four patients were brought to an Emergency facility while twelve remained at home. Only two families reported unexpected excessive sedation. Commonly sited advantages were ease of use without needles, portability, and effectiveness negating an Emergency visit. The most commonly sited concern was a lack of knowing when to initiate treatment. CONCLUSIONS: This preliminary survey supports parental comfort with Diastat gel administration as a safe, effective and easily administered treatment modality for acute seizures in the home.
