Early treatment of status epilepticus

Rationale: To identify if there is a difference in the initiation of treatment for status epilepticus (SE) in patients with and without a history of epilepsy.

Methods: We carried out an observational, case series, analytical, and retrospective study in the Neurology service of the HE CMN Siglo XXI of the IMSS. Information was collected from the clinical records of 40 patients hospitalized for EE from January 2016 to January 2020, obtaining the treatment start time (TST) and history of epilepsy (HE); They were grouped on the basis of HE and TST less than or greater than 1 hour, performing Chi square.

Results: Of the 40 patients included, 57.5% (23) did not have HE, of which 17.3% (4) were treated in less than one hour. Of the patients with HE, 5.8% (1) were treated in less than one hour, with an OR 0.29 with 95% CI 0.03-2.93 (p 0.37).

Conclusions: Epilepsy can be complicated by status epilepticus in 1-16% of patients during their lifetime, being a serious condition, with a high possibility of sequelae. Treatment within the first hour improves the prognosis, which is better with AE. However, in our study there is no difference between TST with or without HE, and TST should improve in patients without HE as they have a worse prognosis; although, the hospital should be considered as a referral center. Further studies are required to corroborate the results.
