Rationale: Catamenial epilepsy (CE) is a periodic increase in seizure frequency in women with epilepsy during menstruation, or at any specific point in the menstrual cycle. Methods: We present two cases who suffered CE and received cyclic progesterone supplement during luteal phase. Results: Patients are 22 and 18-year-old woman who were diagnosed as idiopathic generalized epilepsy with absence seizure. They were on multiple antiepileptic drugs, taken with good compliance and regular menstruation. Their most seizures were exacerbated around menses (days -3 to +3) compared to other days of the month with a frequency of 2 to 3 times per month. They received micronized progesterone 200mg twice per day during luteal phase of her menstrual cycle. Now 12 months after cyclic progesterone treatment, they have had no further clustered seizures around menses, though have some headache and weight gain. Conclusions: The possibility of CE must be thoroughly explored in women with epilepsy and a trial of progesterone therapy should be considered in all women with drug-refractory seizures with catamenial clustering.
