Rationale: Depression is a common comorbidity in both epileptic (ES) and psychogenic nonepileptic seizures (PNES; also known as functional seizures), with traumatic brain injury (TBI) often preceding seizure onset. The negative correlation consistently found between depression and quality of life (QOL) may be mediated by fronto-parietal network (FPN) complexity, as measured by brain Sample Entropy (SampEn). In healthy brain, SampEn is relatively stable, but in a diseased brain, SampEn changes reflect organizational changes. Lower SampEn has been found in people with Alzheimer’s disease, representing decreased neural complexity compared to healthy elderly individuals. We used resting state fMRI (rs-fMRI) to investigate relationships between FPN measures [functional connectivity (rs-FC), SampEn], depression, and QOL in people with TBI-only, ES after TBI (TBI+ES) and PNES after TBI (TBI+PNES). We hypothesized that the negative relationship between depression severity and QOL would be mediated by FPN measures in those with seizure disorders. We also hypothesized there would be direct relationships between FPN measures and QOL in seizure disorder groups.
Methods: Adults with TBI-only (n=60; 50% male), TBI+ES (n=21; 47.6% male), and TBI+PNES (n=56; 25% male) were recruited, completed the Beck Depression Inventory II (BDI-II) and QOL in Epilepsy (QOLIE-31) assessments, and underwent rs-fMRI. Z-scores representing rs-FC strength between FPN regions, and SampEn values representing FPN complexity were calculated per subject. Correlations examined relationships between depression symptom severity (BDI-II), overall QOL score, and both rs-FC z-scores (p < 0.05, FDR corrected) and SampEn values. Mediation analysis was only performed when BDI-II was correlated with both QOL and rs-fMRI measures (rs-FC or SampEn). Correlations examined direct relationships between rs-fMRI measures and QOL.
