Rationale: This project aimed to adopt a Health Care Transition program utilizing the American Academy of Pediatrics, the American Academy of Family Physicians, and the American College of Physicians, (AAP/ACFP/ACP) Got Transition's Six Core Elements of Health Care Transition™ 3.0 clinical practice guidelines which are evidence-driven strategy model for transition implementation. The program helps to improve the ability of youth with special healthcare needs to receive an organized process for adult-centered healthcare transition. Adult care planning models provide ongoing needs management, independence, and consistent clinical strategies for care.

Methods: This project utilized a quasi-experimental design. The independent variable within the DPI project is the AAP/AAFP/ACP Got Transition's Six Core Elements of Health Care Transition™ 3.0 clinical practice guideline implementation. The dependent variable is the referral rate of epilepsy patients over the age of 18. The design allowed for the comparison of transition referral rates before and after implementing the healthcare transition program. The design structure helped to achieve the goals and objectives of the project without disrupting patient care.

Results: The focus of this quality improvement project was on improving the transition planning by assessing patient readiness and increasing referrals into adult care using the AAP/AAFP/ACP Got Transition Six Core Elements of Health Care Transition™ 3.0 clinical practice guideline. The total project included 73 (n = 25 in the comparative group and n = 48 in the implementation group) participants diagnosed with epilepsy between 18 to 30 years old. Data on referrals were collected from the facility’s EMR. The mean age was 20.68 (SD = 1.97) for the comparative group, ranging from 18 to 27. For the implementation group, the mean age was 20.31 (SD = 1.88) with a range from 18 to 27. The referral rate was 24.0% (n = 6) for the comparative group and 64.6% (n = 17) for the implementation group, X2 (1, N = 73) = 10.83, p = .001. The p-value of .001 indicates that the increase in referral rates was statistically significant. Clinical significance was supported by the 40.6% increase in referral rates after the implementation._x000D_
Conclusions: The significance of the QI project supports transition programs that use a standardized process that initiates a known policy for patient transition into adult care to create successful transfers.
