Multiple Subpial Transection. Clinical Outcome in 34 Patients.

RATIONALE: To review the results of multiple subpial transection (mst) METHODS: 34 patients have undergone mst since 1989. Their notes were examined retrospectively for seizure outcome and complications RESULTS: 16 patients underwent mst alone, including 5 with Landau-Kleffner syndrome. These 5 all improved. 8 patients with Rasmussen's disease were operated in 4 there was also a resection.7 of these had some improvement up to Engel 3A or better and one patient remained seizure free (median FU 45/12), but only three did not require a subsequent hemispherectomy. 7 patients with CD underwent mst and 6 of these had a cortical resection. 4 of these had a significant improvement (median FU 51/12). 11 oher patients were operated and 7 had an improvement. In only one patient was there a permanent neurological deterioration. CONCLUSIONS: MST is effective in intractable focal epilepsy and the rate of complications compares favourably with resective surgery on the same area.
