The Role of Magnetoencephalography in Presurgical Evaluation of Patients with Insular/Peri-Insular Cortex Epilepsy

Rationale: Background: Magnetoencephalography (MEG) is suggested as a useful clinical tool to provide additional information for localizing the epileptogenic zone or planning intracranial electrode implantation. Objectve: The study aimed to evaluate the role of MEG in the localization of insular/peri-insular cortex epilepsy (IPICE).  Methods: Data from 20 patients with IPICE who were identified by presurgical evaluation and resective surgery from January 2011 to June 2015 were included and analyzed in the study. Results: The ictal symptoms strongly supported IPICE in 6 patients and MRI supported IPICE in 6 patients. The localizing role of MEG was classified into grade I, II, and III, from slightly suggesting IPICE to strongly suggesting. MEG showed obvious localizing role in 80% of the patients (9 in grade II and 7 in grade III) . In the 14 patients with negative MRI, the MEG were localizing in 11 (78.6%) patients. The original plan of intracranial electrodes planting was modified in 8 patients according to MEG findings. The resective surgery involving the insular/peri-insular cortex was performed in all 20 patients. During the follow-up (1.5 to 6 years), 12 (60%) patients were seizure-free after operation.  Conclusions: MEG played an additional and valuable role in the localization of IPICE. Funding: No
