TREATMENT OF EARLY POSTOPERATIVE HYPOMANIA WITH VALPROATE IN PATIENTS SUBMITTED TO CORTICO-AMYGDALO-HIPPOCAMPECTOMY

One quarter of the patients submitted to cortico-amygdalo-hippocampectomy (CAH) for treatment of refractory epilepsy have psychiatric alterations during the immediate postoperative period and most of them would need brief medical treatment. We report the results of treatment of postoperative hypomania using valproate in the above mentioned patient[rsquo]s population.
Four patients submitted to CAH (1 left, 3 right) who developed hypomania during the first postoperative week were studied. None of them had a previous history of psychiatric treatment and in all of them, hypomania clearly interfered with daily activities.
Three patients were treated with valproate 500mg/day (for 2, 3 and 6 months respectivelly) and 1 with 2000mg/day (still under treatment after one year). In all patients, hypomania symptoms subsided immediately after the start of valproate. Only the patient who received 2000mg/day needed prolonged valproate treatment.
Low-dose valproate treatment is effective in controlling postoperative hypomania after CAH. The majority of the patients would need only low-dose, brief treatment.
