VAGUS NERVE STIMULATION (VNS) THERAPY IN 46 PATIENTS WITH LENNOX-GASTAUT SYNDROME (LGS) IN VENEZUELA

Rationale: To analyze efficacy and safety in patients with pharmacoresistant seizures associated with LGS treated with adjunctive Vagus Nerve Stimulation (VNS) in Venezuela.Methods: 46 patients 27M (59%) 19F (41%) were retrospectively evaluated between December 2005 and May 2013. Mean age: 19.10 +/-8.47 (3 49 years). Mean age at implant: 15.58 +/- 8.07 (3-42 years). Age at epilepsy onset between 3 months and 8 years. Duration of epilepsy by the time of implant: 6.01 +/-2.96 (0.7 10.7 years). Time of follow up: 3.52+/-2.08 (0.5 8 years). Number of AED: 3.41 +/- 0,74 (2 5). Two patients had previous callosotomy. Seizures were counted at baseline and at the end of the study. VNS parameters: Output: 1.54 +/- 0.3 (0.75 2.0 mA), On time: 21.3 +/- 11.15 (7 30 sec), Off time: 3.1 +/-2.2 (0.3 5 min).Results: Mean number of seizures Pre-VNS: 398+/-504 (15 3000/month) Post-VNS: 89 +/- 103 (0 600/month). Forty patients (86.95%) had a decrease of 50% in the frequency of seizure and 26% of subjects (12 patients) had a seizure reduction of more than 90%. One patient (2.17%) is seizure free. Magnet effect YES: 33 (71.7%). Severe side effects were not reported and only one patient (2.17%) had an acute wound infection and the device was removed and re-implanted six month later.Conclusions: 1- Median seizure reduction was 73% at the end of the study. 2- Seizure reduction 50%: 86.95% and 90%: 26% at the end of the study. 3- Seizure Free 2.17% 4- Positive magnet effect was observed in 72% of cases. 5- Severe side effects were not reported.
