VAGUS NERVE STIMULATOR IN A PATIENT WITH LENNOX- GASTAUT SYNDROME- FOUR YEARS FOLLOW UP

Rationale: The Lennox-Gastaut syndrome (LGS) is a severe childhood epileptic encephalopathy characterized by multiple types of intractable seizures, cognitive and behavioral abnormalities and specific electro encephalographic features. Most patients are refractory even with polytherapy, so alternative treatment is indicated. Callosotomy is indicated in these cases, however vagus nerve stimulator (VNS) is a less invasive option.Methods: Case reportResults: GOP, male, 19 years, developed epileptic seizures at 10 months of age. Evolved with uncontrolled seizures, developmental delay and subsequent diagnosis of Lennox Gastaut syndrome. Despite of use of different antiepileptics drugs he still keep having several daily seizures so surgical treatment was indicated. VNS was chosen instead of callosotomy. In the first three months the patient showed reduction of around 20% of seizures with mental performance improvement. After one year of implantation we observed 30% reduction in seizure rate with significant intellectual development already managing to feed himself among other psychomotor progress. After 4 years of implant the patient presents reduction of 40% of seizures rate with maintenance of acquired functions. Conclusions: VNS was equivalent in efficacy compared to callosotomy in seizure control but with improved psychomotor development.
