Rationale: We evaluated the performance of voxel-based analysis of FDG PET images with statistical parametric mapping (SPM) in the presurgical lateralization and localization of epileptic foci in children with intractable epilepsy. Methods: Thirty-one children with intractable focal epilepsy (age: 11 ± 4 years, range: 5-18) underwent interictal FDG PET, followed by 2-stage epilepsy surgery with chronic subdural EEG monitoring. PET images were analyzed visually and using SPM-2 by comparing each subject’s PET images with those of 13 young controls (age: 26 ± 3 years). SPM analysis was performed using significance thresholds of p<0.001 (uncorrected), p<0.0001 (uncorrected) and p<0.05 (corrected). Seizure onset localization in intracranial EEG was used to evaluate the performance of visual and SPM analysis. Left and right hemispheres were evaluated separately and localization was evaluated in 8 brain regions in each hemisphere (lateral and medial surfaces of all four lobes). Results: FDG PET showed focal hypometabolism in all children, both by visual assessment and by SPMp<0.001
