During the last decade, SEEG has become the most popular technique for invasive evaluation of drug-resistant epileptic patients. Though it appears safer and seems to offer greater chance of seizure freedom than subdural evaluation, SEEG does not guarantee surgery or seizure freedom. SEEG needs strong hypotheses before electrode implantation, with a limited number of intracerebral electrodes that sample only a small part of the brain. One of the difficulties facing clinicians is to understand, once the electrodes are in place, whether the seizure pattern recorded at seizure onset is relevant or not, which strongly influences the surgical decision. This webinar reviews situations suggesting that the seizure onset zone has been missed and the cautions and considerations to be taken before deciding to proceed with surgery.
